The Nursing Home scheme was first introduced in March 2013, it meant that each home had a aligned GP and offered nursing homes consistent care for each other their residents.
GP's are required to do weekly visits to the home and also go to the home on an adhoc basis is their is a concern for a patient.
The scheme has really helped to reduce emergency admissions and has improved the quality of care to the residents of these homes.
However we do still have some homes without an aligned GP.
