GPs are encouraged to use the Diabetic Foot MDT pathway for patients experiencing limb-threatening and life-threatening diabetic foot problems. Referral forms can be downloaded from the BHRUT website at www.bhrhospitals.nhs.uk/gps-healthcare-professionals?search-media=diabetes . All referrals must be sent to bhrut.sm-diabeticfootmdt@nhs.net
Use of the MDT pathway will reduce the emergency admissions rate for our diabetic patients
Please note: this is not a replacement to existing NELFT Community Podiatry Services based within Barking & Dagenham, Havering and Redbridge – and referrals should continue in the usual way.
