GP Practices will be sent a "FIT starter pack" from your local pathology lab by 31st May 2019.

Patients given a FIT should also be given a patient information leaflet, detailing practical advice on how to collect their stool sample and expected next steps. GP practices should add patients to their safety netting systems, ensuring that patients are followed up if FIT samples are not returned or if patients do not attend their urgent appointment.

For practices who do not use a centralised upload system, practice managers will need to download the referral form onto the GP IT system. It is best to check with your practice manager to ensure that you have the latest form available.
